Role Overview
As a Primary Care Network Dietitian you will work across two PCNs—Celandine & Metrocare PCN and North Connect PCN—providing 2.5 days of service at each site per week while based in the North Neighbourhood, Ruislip / Northwood.
You will be part of a multidisciplinary team that includes GP staff, community, acute, social and voluntary providers. You will deliver dietetic care directly to patients, manage long‑term conditions, promote self‑care and support the overall PCN mission to bring care closer to the community.
Responsibilities
* Provide specialist nutrition and diet advice, treatment plans and prescriptions to patients, carers and healthcare professionals.
* Educate patients with diet‑related disorders on healthy eating and disease prevention.
* Deliver dietary support to adult patients (over 16) in GP practices, hub clinics, patient homes and care homes.
* Work collaboratively with the multidisciplinary team to secure patient cooperation and understanding of recommended dietary treatments.
* Develop, implement and evaluate a seamless nutrition support service across the PCN, improving standards of patient care and multidisciplinary working.
* Offer recommendations to PCN staff on medication changes for nutritional management based on biochemical, physiological and dietary assessment.
* Implement all aspects of effective clinical governance, including audit, evaluation, supervision and training.
* Communicate effectively with secondary care providers, ensuring appropriate signposting and building close links between services.
* Liaise with on‑site practice staff (reception, nurse, health‑care assistant, GP) as required for ongoing care.
* Maintain accurate patient records using the GP computer system EMIS.
* Use technology to conduct virtual clinics, group sessions and face‑to‑face appointments.
* Undertake home visits for patients unable to attend outpatient services.
* Provide education, training and awareness to the primary healthcare team on dietetics.
* Collaborate with the CCG, commissioners and other stakeholders to ensure consistency of patient care.
* Collect and use user and staff feedback, including case studies, to improve service design and strategic care management.
* Contribute to the development of a primary care–based dietitian service that complements community and secondary care services.
Required Qualifications
* BSc or pre‑registration MSc in Dietetics, approved by the British Dietetic Association.
* Registered member of the Health and Care Professionals Council (HCPC).
* Demonstrated ability to operate at an advanced level of practice.
* Prescribing qualification desirable (optional).
Experience & Skills
* Minimum three years of post‑graduate dietetic experience in community, outpatient, GP and home‑visit settings.
* Experience in triage and care planning.
* Strong communication skills to convey information to patients and multidisciplinary teams.
* Experience liaising with secondary care and using EMIS electronic records.
* Knowledge of NICE guidelines for medication involvement and patient empowerment.
* Proficiency in quality improvement, clinical audit and governance.
