Job Title: Social Worker – Hospital Team

Job Description:
Liquid Personnel is recruiting Adult Social Workers to join its client’s Hospital Team based in Lambeth. The role focuses on promoting strengths-based practice and driving positive change in people’s lives through the use of social work methods, models and tools. The aim is to support individuals in achieving maximum independence while effectively managing risks related to abuse and neglect.
You will be expected to make professional judgements within the framework of relevant legal and policy guidelines, including the Care Act and safeguarding legislation.
What will your responsibilities be?
In this role, you will,
1. Conduct Care Act-compliant assessments of patients’ needs while in hospital.
2. Develop and implement safe, person-centred discharge plans in collaboration with patients, families, and healthcare professionals
3. Work closely with NHS staff, community services, housing, and voluntary sector partners to ensure timely and effective discharge.
4. Attend ward rounds and multidisciplinary team meetings to contribute social care perspectives
5. Manage urgent and complex discharges, including those involving safeguarding, mental capacity issues, or lack of suitable accommodation.
6. Apply the Mental Capacity Act and undertake Best Interest decisions where necessary.

Qualifications & Experience:
To be successful in this role you must have,
9. Social Work England registration
10. Eligible to work in the UK
11. Hold a full UK driving licence
12. Post-qualified experience
13. Degree level or equivalent in Social Work
