As a Patient Safety and Investigation Lead, you will be working both autonomously and in collaboration with the corporate and divisional teams to lead system level patient safety incident investigations.

Work with divisional and corporate teams to identify areas for improvement from patient safety events in line with relevant Trust learning response policies and procedures.

Be involved in continuously reviewing and improving various elements of the Trust's Patient Safety Incident Response Framework policies and plan.

Deliver and support the Trust to provide compassionate engagement and involvement of those impacted by patient safety incidents.

Lead on the systems and processes required to provide oversight of key patient safety compliance metrics, including Duty of candour.

Be jointly responsible for designing and delivering training for a wide range of staff on various learning response methods and patient safety science.

Continue to develop own expertise of patient safety and support the Trust to evolve the capacity and capability of patient safety expertise within the Trust. This includes linking in with national patient safety expert networks.

Lead and continually seek to improve the operational effectiveness of the corporate patient safety team and that resources are optimised to their greatest effect.

Contribute to and participate in the design and delivery of the Trust response to the national patient safety strategy.

Provide expertise in patient safety science including system analysis, just culture, human factors, and the national patient safety strategy, including the patient safety incident response framework (PSIRF).

Provide expertise in patient incident investigation (including SEIPS), as well as other patient safety learning response methodologies. This includes direct observation of care as well as exploring with staff and patients on what occurred with aim of identifying any gaps and improvement areas in provision of care.
