Job Title: Adult Social Worker - Hospital Discharge
Location: Barnsley (Hybrid)
Pay Rate: £35 per hour
Liquid Personnel is currently recruiting for an Adult Hospital Social Worker on behalf of our client. This is an exciting opportunity to join a dedicated hospital discharge team, where you will play a vital role in supporting adults during their transition from hospital to home or other care settings.

Experience working with health partners or hospital social work discharge teams.

Supports informed decision-making processes.

To be successful in this role you must have,
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
