Job overview

35 hours per week

The post holder will provide specialist support to patients with cancer and other life-limiting illnesses. They will be an integral part of the team within the Trust, acting as the first point of contact for patients and multi-professional team members accessing the Specialist Palliative Care service. Responsibilities include delivering specialist advice and ongoing support to their client group.

A professional development plan will be developed to support the post-holder's progression, utilizing a recognized competency framework. This will enable development in leadership, education, and strategic contributions to palliative care within the Trust. Mentorship will be provided by Band 7 specialist nurses, and the post-holder will carry a supervised patient caseload.

Main duties of the job

The post holder will maintain links between the acute sector and primary care teams to ensure smooth patient care transitions. This includes liaising with services within South Tees Hospitals NHS Foundation Trust and other partners.

Although primarily based in the community, the post requires rotation throughout the service, including acute and community settings, and participation in a 7-day service, working weekends and bank holidays on a rotational basis.

Person specification

Qualifications & Training

* NMC Registered Nurse (Adult) Registration
* Evidence of Palliative/End of Life education or similar
* Degree/Diploma in Nursing

Experience

* Relevant post-registration experience at band 5 or above in cancer, palliative/end of life care, or chronic disease management

Knowledge & Skills

* Demonstrable knowledge in Palliative/End of life care

Disclosure and Barring Checks (DBS)

All new candidates requiring DBS clearance will pay the cost via salary deduction from their first month's pay.
