Clinical Nurse Specialist – Advanced Kidney Care
An exciting Band 6 opportunity has arisen at St James Hospital within Renal Services in the Advanced Kidney Care CNS team.
We are looking for a motivated, forward‑thinking registered nurse with the ability to work effectively within a multi‑disciplinary team, demonstrating excellent communication skills.
Main duties of the job
Applicants are required to have:
* Significant post‑registration experience at Band 5 or equivalent, some of which should be within renal medicine.
* Recognised Teaching/Assessment qualification.
* Recognised Non‑medical prescriber qualification, or willingness to undertake within the first 6 months.
* Excellent communication and interpersonal skills.
* Evidence of multidisciplinary team working.
* The ability to work both autonomously, and as part of a team.
There is the opportunity for professional development and an individualised in‑service induction package will be available for the successful applicant, supported by the Renal Unit Clinical Education Team.
Job responsibilities
The post holder will be responsible, alongside a small established CNS team, for patients with chronic kidney disease located across Leeds and the wider West Yorkshire area. The primary focus is to provide comprehensive education and support to patients and their families as they prepare for the next stage of their kidney journey. This includes helping patients understand their condition, exploring treatment options such as haemodialysis, peritoneal dialysis, transplantation and conservative management, and supporting decision‑making. The post holder will also offer guidance on lifestyle adjustments, symptom control, and psychosocial support, ensuring that patients feel empowered and involved in their care. Education will be delivered in a variety of formats, including one‑to‑one sessions, group education, tailored to individual needs. This will be in a variety of settings including patients’ homes (across West Yorkshire), outpatient clinics and inpatient areas.
The successful candidate will deliver evidence‑based care through independent assessment, care planning, treatment, and evaluation for patients with advanced kidney disease, in line with agreed protocols. The role includes managing nurse‑led clinics, both face‑to‑face and via telephone, supported by Consultant Nephrologists.
