Overview
Surrey Downs Heath & Care - Epsom PCN. This is an exciting opportunity for a highly motivated and enthusiastic Healthcare Support Worker to work within the community nursing team at Leatherhead PCN. The post-holder will provide care and support to patients in the community setting, undertaking a range of delegated tasks. You will be supported to develop skills and abilities to enhance your career and provide appropriate care to patients. We are a supportive team that welcomes applicants with or without community experience to contribute to nursing in the community. The role offers the support, training, autonomy and flexibility of a modern NHS service and is suitable for those with an interest in pursuing a health career.

Responsibilities
Main duties of the job
* Clinical:
o Coordinate and participate in specialist care (e.g., wound care, Doppler testing, phlebotomy, bladder screening and clinical activities) under the supervision of Registered Nurses
o Undertake diagnostic tests on request of District Nurses, Community Matron or GPs, including home visits or coordinating repeat/regular testing for residents in residential homes
o Undertake clinical interventions as delegated by a senior team member
* Health Promotion:
o Participate in the assessment and implementation of holistic care programmes within set standards
o Support, educate and inform family/carers where appropriate
o Assist in promoting health and wellbeing of patients
* Administration And Clerical Duties:
o Maintain clear, signed, contemporaneous records according to Surrey Downs Health and Care procedures
o Take and record telephone messages and liaise appropriately
o Manage stationery and documentation supplies
o Maintain data entry and other record keeping (e.g., recording mileage and annual leave on staff databases)
* Team Working:
o Work as part of the team, adhering to agreed standards relevant to the role
o Assist with clerical and administrative duties as required
o Maintain regular contact with the senior team member and communicate relevant issues
* Other Duties:
o Maintain patient privacy and dignity
o Maintain liaison with the senior team member responsible for workload
o Co-operate with other professionals and agencies involved in care
o Be aware of and act on procedures to protect vulnerable individuals
o Participate in Surrey Downs Health and Care Governance and Risk Management strategies
o Undertake duties reasonably required that are consistent with the post
Person Specification
Qualifications
* NVQ Level 3 or appropriate qualification or course
* Full-Current UK driving licence
Skills
* Ability to provide quality care responsive to patient needs and to work in partnership with patients, clients and other agencies
* Good written and verbal communication
* Ability to work effectively as part of a team
* Ability to drive and have access to a vehicle for work
Experience
* Essential: Experience of working directly in a care coordinator role, adult health and social care, learning support or public health/health improvement
* Desirable: Significant administrative and clinical experience
