Band 5 Community Staff Nurse - Hawthorns - West locality
Band 5 Community staff Nurse working with the Hawthorns Team within the West locality based at Soho Road Health Centre, B21 9RY.
Providing holistic evidence-based care to housebound patients. You will be visiting patients in their own home to deliver all aspects of nursing care, you will therefore need to have a UK driving licence and access to a car to use for work purposes.
Job Overview
The post holder will work as part of the community nursing team under the supervision of the Team Manager. He/She will be accountable for the delivery of high-quality care to patients within their own homes and the community nursing clinic at times.
Main Duties
* Working with other members of the team to provide care to and promote health/wellbeing and independence for patients on the team caseload.
* Working closely with other partners/stakeholders including GP’s, tissue viability and vascular clinics.
Person Specification
Professional Qualifications
* Registered Nurse Level 1 (Adult)
* Mentor and assessor qualification or equivalent
* Evidence of continued professional development relevant to role
Experience
* Experience of working in a multi-disciplinary team setting
Skills/Knowledge
* Ability to demonstrate sound knowledge and application of up-to-date evidence-based clinical practice.
* Excellent communication and analytical skills and a problem-solving approach
* IT literate
* Awareness of the complex needs of a multicultural, socially diverse population.
* Effective time management
