The aim of Adult Community Services is to provide a first-class healthcare service across in-patient and community services. We serve a broad multicultural community throughout Hackney and the City. We offer excellent training, development, and support to enable our staff to meet high healthcare standards. We have a vacancy for a Deputy District Nurse Team Leader within the Adult Community Services Department to provide nursing care to patients in their homes.

Main duties of the job
The post holder will be responsible for planning, implementing, and evaluating nursing care needs and delivering high-quality, up-to-date care to patients in their homes and other settings. They will work within a team for a defined number of patients, under the supervision of the Clinical Operations Manager. The post holder will manage a caseload in the absence of the Clinical Operations Manager/District Nurse and will collaborate closely with GPs to advocate for and communicate about patient care.
