What the Job Involves As a Social Care Assistant, you will assess the needs of service users in accordance with the Care Act, developing strengths-based support plans to maximize independence. You will collaborate with health and social care colleagues, conduct risk assessments, organize assistive technology solutions, and provide ongoing support for rehabilitation. The role requires managing an individual caseload, facilitating transitions to appropriate accommodations, and navigating complex social care scenarios. You will maintain accurate records, contribute to case conferences, and ensure safeguarding interventions are effectively actioned.
