What you will do
* Undertake person‑centred assessments, develop care plans, and complete reviews for adult service users
* Work collaboratively with partner agencies and professionals to deliver a holistic approach to community care
* Provide advice, support, and guidance to families and carers, signposting to appropriate services
* Maintain accurate, timely, and comprehensive records and reports in line with departmental requirements
* Travel across Helensburgh and Lochside to visit service users and attend meetings as required
