Job Description
We have an exciting opportunity for a Care Coordinator to join our team in Inverclyde supporting our Care Professionals and Clients across the area.
1. Build and maintain positive relationships with clients, their families and other professionals involved in their care.
2. Support the Registered Manager to forward plan the care schedules to ensure continuity of care.
3. Be responsive to changes in the schedule and consult with the relevant team members.
4. Support care consultations, service reviews and completion of client personal care plans.
5. Conduct introductions, supervisions and touch points with Care Professionals
6. Supporting care visits by covering as and when required.
7. Completing regular audits to ensure high quality care is maintained.
8. Coordination of on-call rota and other events on behalf of the office team.
9. Monthly participation in on-call duties.
10. Ensure compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
11. Perform any other duties deemed necessary for the successful operation of the business.
Qualifications
12. Must have previous experience in a co-ordinator or scheduler role.
13. Care experience.
14. Excellent organisation skills.
15. Good IT skills.
16. Genuinely passionate about delivering high quality care.
17. Driver and have access to own vehicle.

18. 35 hours per week (09:00-17:00) Monday-Friday
19. £26,724 plus bonus scheme.
