Overview
This is an exciting opportunity for a motivated paediatric nurse with an interest in Asthma to work with Children and families in the community. We are looking to recruit an Asthma Specialist Nurse to manage nurse-led clinics across the community within Enfield GP practices.
The Asthma nurse service is intended to provide more specialist skill in the community, supporting primary care and decreasing need for referral into secondary care for management. This will include nurse led clinics in GP surgeries, community centres, school and home visits. As well as delivering improvements in consistency and quality of care in the community it will decrease both outpatient activity, ED activity and primary care activity through better control and access to more specialist input in the community. The post holder will work collaboratively with all members of the Multidisciplinary team including GP’s, practice nurses, community children’s nurses and school nurses to deliver effective, high quality, safe patient centred care in the community and on the main Hospital site.
This established service further provides expert specialist skill in the community, support primary care, and decrease the need for referral into secondary care for management. As well as delivering improvements in consistency and quality of care in the community it will decrease outpatient activity, emergency department activity and primary care activity through better control and access to more specialist input in the community.
