You possess at least three years of post-qualified experience in Adult Social Care and have proficiency in line management and supervision. You are experienced in conducting person-centred, asset-based, outcome-focused assessments, care management, and reviews. You hold a strong knowledge base about adult social work, with a commitment to personalized, outcome-focused practice and a deep understanding of social care legislation and practice. What the Job Involves The role involves making a real difference in people's lives every day through your work in Adult Social Care. As an experienced Senior Practitioner, you will manage a small caseload while being supported to make evidence-based decisions. You'll work as part of the locality management team to support performance and lead practice. The role offers opportunities to practice with a variety of customer groups, working with individuals with mental health, learning disabilities, physical disabilities, frailty, and safeguarding needs. Additionally, you will be supported in developing specialisms and maintaining professional development. Given the rural nature of Herefordshire, the ability to travel is crucial.
