TITLE: Care and Support Worker
REPORTS TO: Care and Support Manager
HOURS: We are open to discussion but ideally we are looking for the following:
Position 1: Total 19.5 hours (Monday and Tuesday 9:15-15:30, Wednesday 9:00am – 4:00pm)
Position 2: Total 13.25 hours (Wednesday 9:00am – 4:00pm, Thursday 9:15-15:30)
BASE: Homelink Centre
PURPOSE OF THE ROLE
Care and Support Workers act as Link worker for a defined caseload of clients. The postholder is responsible for delivering person-centred care and support to attendees of the Homelink Day Centre, who are affected by a range of conditions including but not limited to dementia, neurological conditions, or mobility issues. This individualised support should reflect the personal interests and needs of the client, as described in the client's care plan.
MAIN RESPONSIBILITIES
1. Act as a Link worker, developing individual care plans, processing new client information and providing co-ordination and continuity of care and support for a range of clients allocated to you.
2. Provision of respectful personal care in accordance with the needs, wishes and preferred routines of each individual client as identified in their care plan. This includes assistance with eating and drinking, using the toilet and managing continence aids and well as supervising people to administer their own medication.
3. Planning, organising and delivery of purposeful activities, tailored to the needs, interests and abilities of individuals, as well as group activities that will bring individuals together.
4. Promote the self-esteem, enjoyment, and emotional health of clients, encouraging and supporting them to express their needs views and concerns.
5. Develop and sustain positive trusting relationships with clients and their carers e.g., liaise with carers at the end of the day.
6. Write clear, accurate daily client record notes to reflect the session spent by each individual in our care.
7. Work as a coordinated team as directed by the Centre Coordinator to deliver bespoke services.
8. Maintain a clean and tidy environment in line with all relevant health and safety, fire safety and individual client safety plans, procedures, and policies. Report any hazards as appropriate.
OTHER RESPONSIBILITIES
1. To take responsibility for an agreed area of 'designated responsibility' as appointed to you by the Care and Support Manager / Centre Coordinator.
2. Attend and contribute to team meetings, working groups, individual supervision, and appraisals.
3. Participate in and contribute to training and development.
4. Report any concerns/risks/issues immediately to Centre Coordinator / Care and Support Manager as appropriate to seek advice and support.
5. This job description is a representative document. Other similar duties may be allocated from time to time commensurate with the general character of the post and its grading.
6. Where necessary assisting with duties usually completed by volunteers e.g. serving refreshments and the setting of dining tables.
PERSON SPECIFICATION
Education and Qualifications
Good standard of general education.
Relevant qualification in health and/or social care.
Experience
Previous experience in a care and support role.
Experience of delivering individualised activities.
Experience of working with people with dementia.
Experience of working with older people with complex needs.
Experience of working with vulnerable people in a charity or other community group
Proven track record of successfully working as part of a team.
Previous experience in a similar role.
Experience of working with carers.
Skills, Abilities and Knowledge
Excellent communication skills.
Understanding of the issues affecting older people, particularly in relation to people with dementia and their Carers.
Ability to continuously assess and reassess client support needs within the Day centre setting, to be able to make informed decisions around client safety, moving and assisting and general wellbeing.
Knowledge of services available within the local area

· All staff are asked to sign a confidentiality agreement to protect Homelink, its staff, Clients, Carers, Volunteers and Trustees. Any breach of confidentiality will be taken seriously and may result in disciplinary action.
· There may be an occasional requirement for the postholder to work flexibly, weekends / evenings.
· Any offer of employment will be subject to an enhanced DBS check and two successful references.
