About the Hospice at Home team:
Our Hospice at Home service brings the skills and expertise of our highly trained and dedicated specialist team directly into our patient’s homes. This allows patients, their families, and carers access to the vital support they need at home, at a time when it really matters.
About the role:
We are looking for an experienced and versatile Triage Specialist Nurse with excellent communication and interpersonal skills, to join our Hospice at Home team. You will work alongside the Triage Lead managing referrals efficiently, enabling the Princess Alice Hospice at Home team to be responsive to patients and carers in a timely way.
About you:
You will be comfortable acting as a specialist advisor for other healthcare professionals, patients, and carers. You’ll enjoy liaising directly with different referrers (including the Primary Care team) so you can ensure patient needs are holistically met.
You will feel confident in holding a telephone caseload of non-complex patients to help support the Clinical Nurse Specialist team in caseload management.
You will have a relevant degree, diploma or be in the process of studying for one. Palliative care or oncology experience is essential to ensure a solid understanding and relevant clinical knowledge for you to draw upon.
