Senior Health Care Assistant - Apprenticeship Opportunity

The closing date is 15 June 2026.

This post will work across both the Well‑being team and the older people’s mental health services to support monitoring of medication and physical health checks for an identified cohort. The Community Wellbeing Team (SMI Health Checks) provides a health and social care service for people experiencing or recovering from severe and enduring mental health problems.

The post holder will undertake an annual Physical Health Check as detailed in the QOF guidance for people with a severe mental illness (SMI). They will support people to develop and achieve the outcomes of their Health Action plan.

If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH offers this post alongside the opportunity to complete a Level 3 apprenticeship qualification relevant to your role. As an apprentice you will gain experience, learn work‑related skills and obtain tools needed to thrive. The apprenticeship includes regular training with a college or other organisation; around 20 % of your working hours will be spent training or studying, leading to a nationally recognised qualification. Whether you already work for us or are joining the Trust and NHS for the first time, apprenticeships can help you to develop the skills, knowledge and know‑how to embark on a successful and fulfilling career within the health service.


Main duties of the job

* Undertake an annual Physical Health Check as detailed in the QOF guidance for people with a severe mental illness (SMI) and ensure all interventions are recorded in the Trust Electronic Patient Record (EPR).
* Support people to develop and achieve the outcomes of their Health Action plan, working with other agencies to ensure the highest standard of patient care.
* Be accountable for delivering appropriate and effective evidence‑based care to meet the needs of the individual.
* Apply motivational interviewing techniques when planning health interventions.
* Work collaboratively with patients and their carers to facilitate and promote rehabilitation and recovery, helping patients to achieve and maintain their optimum level of mental health functioning and quality of life in the community.
* Undertake memory medication monitoring appointments.


Person Specification


Qualifications

* English and Maths at level 2 or equivalent (GCSE 9‑4 or A*‑C or functional skills level 2).
* NVQ Level 2 in a related subject.
* NVQ Level 3 in a related subject or willingness to undertake a relevant Level 3 apprenticeship.


Experience

* Demonstrate a keen interest caring for others.
* Previous experience of caring for patients in a clinical setting.
* Previous experience of working as part of a team.
* Understanding of the assessment and management of risk within the client group.
* Experience in undertaking ECG and venepuncture procedures and willingness to undertake relevant training.


Knowledge

* Understand importance of maintaining confidentiality.


Skills

* Motivated, enthusiastic, organised team player.
* Able to use own initiative and work without direct supervision within the framework of service policies.
* Ability to prioritise workload depending on variability and unpredictability of patient dependency.
* Recognition of when to seek advice from senior colleagues.
* Skilled in moving and handling patients, objects/equipment following Trust policy and local assessment.
* Basic IT skills and the ability to work in an agile way, using a laptop device to access the patient record.
* Able to make non‑judgemental decisions.
* Maintain, update and develop personal, professional knowledge and skills, achieving high standards of patient care at all times.
