What You Will Do
* Manage a caseload of up to 14 families, providing tailored support within the Family Help Team.
* Undertake comprehensive assessments to understand needs, strengths, and risks, building individualised support plans.
* Collaborate with multidisciplinary professionals to coordinate interventions and ensure cohesive support for families.
* Maintain accurate, timely, and quality case records in line with statutory expectations and best practice.
* Help families access community resources, empowering them to achieve positive change and sustainable outcomes.
* Have 2–3 years’ experience working with children and families within a Community Wellbeing Team (CWDT).
* Strong communication and interpersonal skills, with the ability to build rapport with diverse families.
* Proficiency in social work case management systems and digital tools for accurate record keeping.
