We are looking for a dynamic and enthusiastic clinical pharmacist to join us working with Healthier Neighbourhoods PCN in Wirral. You will join an experienced team to provide clinical pharmacy services to our local population.
Responsibilities
* Support PCN with the delivery of safe and effective systems for medicines optimisation, reducing medicines waste and maximising patient outcomes through structured medication review.
* Contribute to new ways of working and using the skills that your role can bring to evolving PCN services.
* Promote good practice in line with clinical and operational guidelines and clinically support pharmacy technicians with medicines reconciliation post discharge and other medicines projects.
* Spend time within the PCN hub and in GP practices to answer clinical queries from patients, local GPs and clinicians, trusts and other services such as care homes; build strong relationships with community pharmacy and trust pharmacy colleagues.
* Support our patients to help them get the most out of their medicines and to play an active part in their own healthcare decisions.
