Salary:In the range of £45,000- £52,650 WTE dependant on experience.
Working Hours:30-37.5 hours per week, on-site 4-5 days a week with travel across the county for site visits and meetings when required.

We are seeking an experienced and highly motivated Primary Care Network (PCN) Manager to join our dynamic team. The PCN Manager will be responsible for the coordination, management, and strategic development of our Primary Care Network, ensuring the effective delivery of healthcare services to our patients. This position will work closely with healthcare professionals, local organisations, and stakeholders to drive the success of the network and ensure high-quality, patient-centred care.
* Responsible for developing, along with the ACD, the business plan for the PCN to achieve shared vision and agreed outcomes
* Network and benchmark with other PCNs to share and implement best practice
* Responsible for developing the organisational structure for the PCN; this includes workforce planning and agreeing this with the commissioners
Main duties of the job
* Responsible for the recruitment, induction, training and management of all Network staff.
* Responsible for developing and implementing effective and efficient services to meet the DES requirements. Ensuring project plans are in place and monitored to ensure key milestones are met
* Responsible for developing business cases for potential new services to be delivered by the Network
* First point of contact for Practice Managers and GPs regarding delivery of local projects and services.
* Manage contractual requirements, legal and financial requirements of the organisation.
* Co-ordinate the delivery of all Network services and their reporting submissions
* Set up data collection, analyse data and collate feedback for reporting requirements both within the PCN and externally, where required. This should demonstrate contractually compliance and performance improvement.
* Key member of project planning and implementation of all new models of service delivery
* Plan agendas, speakers, ensure all actions are followed up and completed in a timely manner.
* Responsible for communication across the PCN practices and liaison with patient representatives.

Primary Responsibilities:
Leadership and Management:
Lead and manage the PCN team, providing guidance, support, and fostering a collaborative, high-performing environment. Oversee day-to-day operations and ensure the delivery of efficient and effective services.
Strategic Planning:
Develop and implement long-term strategies for the PCN, ensuring alignment with organisational goals and healthcare policies. Work to continuously improve care delivery models and outcomes.
Build strong working relationships with local healthcare providers, community organisations, and stakeholders. Act as the main point of contact between the PCN and external partners.
Project Management:
Oversee key PCN projects, ensuring they are delivered on time, within budget, and in line with regulatory requirements. Monitor project progress and adjust plans as needed.
Quality and Performance Monitoring:
Regularly assess and monitor the quality of services provided within the PCN. Lead efforts to enhance patient care quality, safety, and satisfaction. Collect and analyse data to inform decision-making and quality improvement initiatives.
Budgeting and Financial Management:
Develop and manage the budget for the PCN, ensuring financial resources are allocated efficiently. Monitor and control expenditure, ensuring financial compliance with organizational policies.
Regulatory Compliance:
Ensure that the PCN adheres to relevant healthcare regulations, standards, and policies. Stay informed on regulatory changes and implement necessary adjustments to keep the network compliant.
Training and Development:
Identify training needs for staff within the PCN and coordinate professional development opportunities. Support the recruitment, onboarding, and retention of team members.
Work with the clinical and non-clinical teams to enhance the patient experience within the PCN. Gather patient feedback and implement strategies to improve patient satisfaction and care outcomes.
