Rehabilitation Assistant – Integrated Community Stroke Rehab Team
37.5 hours per week – Join a dedicated stroke rehabilitation team based at Kendray Hospital, Barnsley. The role supports patients following a stroke across inpatient and community settings, delivering high‑quality, evidence‑based rehabilitation to optimise recovery.
Job Summary
The Integrated Community Stroke Rehabilitation Team provides intensive therapy for up to six months after discharge. As a Rehabilitation Assistant you will work closely with nurses, physiotherapists, occupational therapists and speech and language therapists to help patients regain independence and confidence.
Main Duties
* Work as part of a multi‑professional team in both inpatient and community settings, delivering safe and compassionate planned care.
* Assist therapists in the execution of individual rehabilitation programmes, including mobility, self‑care and communication exercises.
* Carry out clinical observations and record vital signs, feeding, and mobility milestones as required.
* Support and motivate patients to actively participate in therapy, encouraging progression and goal achievement.
* Provide personal and therapeutic support throughout the rehabilitation journey, including patient, family and carer education.
* Assist in clinical supervision and ongoing training, applying reflective practice to improve care delivery.
* Use a vehicle to travel to community appointments; a current driving licence and vehicle access during the working day is essential.
* Maintain vaccination status in line with NHS Trust policy.
Person Specification – Essential
* Excellent customer care and communication skills.
* Resilience and the ability to manage pressure, exposure to emotional or physically challenging situations.
* Good IT and computer literacy, including data protection handling.
* Understanding of diversity, cultural competence and trust responsibilities.
* Strong interpersonal, organisational and problem‑solving abilities.
* Experience in providing personal care and routine procedures for people with health or social needs.
* Capability to work independently or as part of a multidisciplinary team.
* Current driving licence, with access to a car during the working day; reasonable adjustments are considered for disabled applicants.
* Willingness and ability to attend all required trust‑mandated and refresher training, including health‑ and safety‑related courses.
Person Specification – Desirable
* Experience of supervising or inducting new staff.
* Knowledge of stroke rehabilitation and related clinical guidelines.
* Additional language skills beyond English.
* Previous training in aggression management or advanced patient handling.
Qualifications
* GCSE level 3 or equivalent in Maths and English.
* Health and Social Care apprenticeship level 3 or equivalent (NVQ or RQF).
* Care Certificate or commitment to achieve within 12 weeks of commencement.
* Level 4 units in Health & Social Care are desirable.
Eligibility and Support
* Health & Care Worker visa sponsorship may be considered if the prorated salary meets the £25,000 threshold, as per UK Visas & Immigration regulations.
* Disclosure and Barring Service check and Rehabilitation of Offenders Act (Exceptions Order) 1975 compliance required.
Salary & Benefits
* Band 3 – £24,937 to £26,598 per annum.
* Permanent contract.
* Full‑time schedule with flexible working options.
