* A copy of your CV or cover letter (ideally CV)
* Information about your past employers. Please can you briefly explain any gaps in employment longer than 2 weeks.
* Details of employment references covering at least the last 5 years, where you don't have 5 years of employment history references can be sought from school/volunteer/personal reference (personal references can't be a family member)
* If you have worked in care previously we require reference details for ALL previous care roles
