Primary Care Network - Mental Health Practitioner

We looking to recruit a Primary Care Network (PCN) Mental Health Practitioner (MHP) to join our existing team of MHP's.Working within GP surgeries across South Norfolk with the SNHiP PCN.

As PCN Mental Health Practitioner (MHP), you'll play an incredibly important role in transforming community mental health services.

We're working collaboratively with the Integrated Care System to offer Place Based Care - ensuring communities receive fast and effective treatment, in the familiar surroundings of their GP surgery.


Main duties of the job

SNhIP PCN are a group of GP practices amongst 3 PCNs network across South Norfolk who work together in a local geographical area, with varying sizes, patient populations and mental health priorities.

As a Primary Care Network (PCN) Mental Health Practitioner (MHP), you will work across the interface between Primary Care and Secondary Mental Health Services as part of a multidisciplinary team providing advice, support, and treatment for members of our community experiencing problems with their mental health and wellbeing.

Mental Health Practitioners in general practice aim to provide advice and support for people with complex mental health needs (such as bipolar, psychosis, eating disorders, severe depression & anxiety, and a range of other mental illness) and the primary care teams involved in their care.

The aim is to enable people to access specialist mental health interventions when required but also to support people to live well in their communities by focusing on strategies to promote mental and physical well-being and prevent deterioration in people's health.

Job responsibilities

From 2025, each PCN will have three Mental Health Practitioners working in its practices alongside a multi-disciplinary team. Practitioners can include Registered Nurses, Social Workers, Occupational Therapists and Clinical Psychologists. On a day-to-day basis, PCN Mental Health Practitioners will work as a member of GP practice extended teams and be based in practices.

You'll work with the existing MHPs in the PCN providing support, treatment, advice, education, and problem-solving approaches to members of our community experiencing challenges with their mental health. You'll be providing a dedicated growing service that makes sure individual needs are met in a seamless environment.

This is a full-time opportunity, and we're flexible and are happy to discuss different working patterns, provided these are within and around the GP practice working and enhanced hours.

The role of the PCN Mental Health Practitioner has already proven itself and has shown to reduce referrals into secondary care.

Person Specification


Qualifications

* Registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker with the relevant professional registration.
* Practice Assessor and Practice Supervisor training (NMC Registrants) or Practice Educator (AHP Social Worker Registrants)
* Ongoing Professional Development working towards degree level (if professional registration is following a diploma level course)


Skills

* Computer skills and experience of using Electronic Patient Record System
* Effective analytical and decision-making skills
* Care planning and risk assessment knowledge and skills
* Able to time manage and reflect on clinical practice.
* Prevention and Management of Suicide training


Knowledge

* Ensure care is values-based, nonjudgemental and equitable, and supports the principles of equality, diversity and inclusivity being met.
* Have the ability to discuss, identify and escalate safeguarding issues in a sensitive way to protect those who are vulnerable.


Experience

* Able to demonstrate an appropriate level of knowledge of mental health practice from previous experiencerom previous experience
* Experience of managing a caseload and the provision of Mental Health Care services to people with mental health care needs.
* Experience in the delivery of clinical interventions.
* Experience in delivering group-based interventions.
