Working at a clinical and leadership level to coordinate the delivery of patient care and achieve KPIs within the aims of the role and the wider Epilepsy Specialist Nurse Pilot.
The post holder will support the Epilepsy Clinical Lead and their team to ensure that their workstream of programmes is planned and managed effectively, and will take the lead in assisting with their successful delivery.
Responsibilities include managing a complex caseload of CYP with epilepsy who require care coordination across multiple settings and services.
Depending on experience and team discussion, the post may involve leading a specialist area of epilepsy aligned with the pilot's priorities, such as transition, medical complexity, or learning disability and autism.
The role involves undertaking system-level responsibilities to support integrated care, including:
1. Establishing referral pathways into services like mental health care, tertiary neurology, and neurodevelopmental services.
2. Supporting the transition of YP into adult epilepsy services, especially for those with complex epilepsy requiring multidisciplinary care.
3. Liaising with community paediatric services for CYP with learning disabilities and autism, including establishing outreach clinics in residential schools.
4. Developing sustainable training models for schools in emergency medication use.
5. Supporting consent processes for genomic investigations.
