You possess a thorough knowledge of health, housing, and social care legislation and are registered with the HCPC. A UK driver's license is required for visiting clients. What the Job Involves As an Occupational Therapist within the Reablement Team, you will manage a complex caseload, conducting strengths-based assessments and developing innovative reablement plans for clients. You will liaise with multi-disciplinary teams to coordinate interventions and promote independence among clients. Your role includes offering professional supervision and support to colleagues, managing client risk, conducting home assessments, and providing safeguarding advice. Active in personal professional development, you will document client interactions and participate in council activities, ensuring all practices meet legal and professional standards.
