* Working Pattern: 35 hours per week
About The Role
We are currently recruiting for an experienced Adult Social Worker to join a dynamic Adult Social Care team in Liverpool. This is an excellent opportunity to work within a forward-thinking local authority, delivering high-quality, person-centered services to adults with a range of care and support needs.
You will play a key role in safeguarding vulnerable adults, promoting independence, and supporting positive outcomes through effective assessment and care planning.
Key Responsibilities
* Manage a varied and complex caseload within Adult Social Care
* Undertake Care Act assessments, reviews, and support planning
* Lead and contribute to safeguarding enquiries
* Complete Mental Capacity Assessments and Best Interests decisions
* Work collaboratively with service users, carers, families, and multi-agency partners
* Develop outcome-focused, person-centred care plans
* Maintain accurate and up-to-date case records in line with statutory requirements
