Practice Nurse – Reading

Location: Reading, UK.
Schedule: Monday and Wednesday 08:00‑18:00. Start ASAP.
Responsibilities
* Implement evidence‑based processes for managing patients with long‑term conditions, including care for elderly and housebound patients.
* Deliver nursing services as a senior member of the multidisciplinary team.
* Provide care in infection prevention and control, health promotion, chronic disease management, well‑woman and well‑man clinics.
* Support the practice management team in reviewing and delivering clinical policy and procedure.
Essential Qualifications
* Registered Nurse with the Nursing and Midwifery Council.
* Experience in chronic disease management (diabetes, hypertension, respiratory conditions).
* Previous practice nurse experience.
* Qualified smear taker.
* Immunisations/Vaccinations trained, including childhood immunisations.
* Clear DBS, right to work, etc.
Desirable
* Experience using EMIS.
* Diabetes qualification.
* Asthma/COPD qualification experience.
* Travel health knowledge.
* V300 prescriber.
