Job Overview
The main role of this post is to provide coordination of the patient pathway and multidisciplinary team for the specialist Neurorehabilitation and PDOC (Prolonged Disorder of Consciousness) services across Wales.
Responsibilities
* Collect, record, analyze and report statistical data as required by JCC, the Health Board's Commissioning Team and nationally.
* Provide an accurate and timely account of the entire patient pathway, tracking the progress of patients from referral to eventual discharge destination.
* Communicate and liaise with key personnel along the pathway, recording accurate information.
* Act as a single point of contact for specialist neurorehabilitation and PDOC services.
Qualifications
* Part‑time role (35 hours).
* Excellent communication and IT skills.
* Ability to work well within a multi‑disciplinary team.
* English and/or Welsh speakers are equally welcome to apply.
