Family Hubs are part of Kent County Councils offer to support to children and young people, aged 11-19 (SEND 25) and their families.

Our local Family Hubs offer a broad and integrated range of early help services for families, which include midwifery, health visiting, parenting support and activities for children and young people.

We have 56 Family Hubs across Kent, where we operate alongside our partners to support local communities.

An exciting opportunity has arisen to join the newly formed Family Hub team in Gravesham District. The successful candidate will operate as a Family Hub Practitioner within the Family Hub team in an agile fashion at a variety of outreach settings (including home visiting) within a district providing 1:1 and group support to children, young people and their families.

The role will work closely with integrated childrens services team and with a range of multi-agency partners including schools, police, district council and community organisations to provide support for vulnerable children, young people and their families.

The post will facilitate and support peer groups and be knowledgeable to support and encourage children young people and their families to takeadvantage of the Family Hub Digital and Virtual offer and other routes of advice and guidance. They will be able to provide expert guidance to families about baby and child development to enable parents to support their child as they grow and achieve in health and education outcomes.

Youwill be expected to work flexibly across the 0-19.

The role requires you to operate a whole family approach in your practice and work around the times of the family. As such, this post will be expected to work flexibly including, on occasions, at evening and weekends, and will be co-ordinated by management as required.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
