Focused Care Pathway Practitioner

Main duties of the job
* Participate in weekly team meetings, communicating service user needs.
* Assist in prioritising workload allocation.
* Work in a focused way with patients to achieve goals identified by the patient.
* Contribute to the weekly multi-disciplinary meeting where service users presenting with an increased level of need are discussed and present cases on a rota basis for team review.
* Formulate care plans in line with identified needs and review them according to the Trust Care Programme Approach and Care Planning Policy.
* Provide clinical supervision and caseload management to support workers.
* Provide therapeutic interventions in line with the agreed care plan and risk assessment, utilizing psycho-social interventions and CBT-based treatment.
* Assess risk, develop risk management plans, review and adjust in line with service user needs.
