Job Overview
This Fixed term/Secondment until 05/10/2026 to cover maternity leave is for a Band 4 Therapy Assistant to join the discharge to recover and assess (D2RA) and Assessment and Discharge Team (ADT) service in the Central area of BCUHB. The service aims to facilitate early discharge from hospital and prevent admission to hospital. You will work with patients with a wide variety of diagnoses, providing support to the Occupational Therapists and Physiotherapists to provide assessment and intervention in patients’ home environments. The team comprises Occupational Therapists, Physiotherapists and generic therapy assistants, and you will work closely with hospital and community multidisciplinary teams.
Main duties of the job
* Significant current experience of working within acute/community services is essential.
* Support development of the service while delivering person-centred care in a timely manner to patients in their own homes.
* Deliver routine physiotherapeutic treatment programmes and delegated occupational therapy intervention plans independently, progressing treatments within guidelines and protocols; you will be accountable for these tasks.
* Some day-to-day administrative duties within the department; proficient computer skills are essential as documentation is electronic.
* Based at Glan Clwyd Hospital with the successful applicants providing support across Conwy and Denbighshire.
* Welsh language skills are desirable; Welsh and/or English speakers are welcome to apply.
