Job Summary
Weaver Vale Practice is seeking a Practice Nurse (or Nurse Practitioner) to work full‑time (part‑time considered). The successful candidate will support the current Practice Nurse, GPs and administrative staff in delivering high‑quality patient care across a range of clinical duties. Experience in chronic disease management, the Quality and Outcomes Framework (QOF) and relevant clinical procedures is essential.
Eligibility & Qualifications
• Registered General Nurse (RGN)
• Experience working in Primary Care
• Knowledge of QOF, chronic disease management, child immunisations, cytology, women’s health, travel health, sexual health and spirometry.
• Ability to travel for home visits, flexible working and excellent organisational and communication skills.
Key Responsibilities
* Chronic disease monitoring in line with QOF requirements
* Administer cytology, urinalysis, travel clinic, ECGs, asthma/peak flow readings, spirometry, antihypertensive management and routine immunisations
* Provide contraceptive advice and support
* Perform minor surgery procedures (joint injections, IUD insertion/removal, Implanon insertion/removal)
* Maintain accurate clinical records, labelling and submission of samples
* Actively participate in practice meetings, audit, training and quality improvement initiatives
Other Duties
* Chaperone patients during examinations
* conduct home visits for chronic disease monitoring and flu immunisation
* Maintain cleanliness and supplies of clinical rooms, order and manage stock and equipment
* Assist with patient education, health promotion and safeguarding responsibilities

Salary will depend on experience and qualifications. This is a permanent, full‑time role. The practice offers a supportive teaching environment with access to an on‑site pharmacy, district nurses and health visitors.
