In This Dynamic Role, You Will
* Assess Needs: Conduct thorough needs and risk assessments in line with the Care Act 2014 and Mental Capacity Act 2005.
* Build Relationships: Foster strong connections with individuals and their families to understand their unique situations.
* Coordinate Support: Work collaboratively with social workers, occupational therapists, and other agencies to provide comprehensive care.
* Connect Communities: Link individuals with local services and resources to promote self-directed support.
* Respond to Inquiries: Address questions and concerns from clients and their families with empathy and professionalism.
To Excel In This Role, You Should Have
* A relevant qualification in social care or a related field.
* Experience in adult social care or community support.
* Knowledge of legislation affecting social care, including the Care Act 2014 and Mental Capacity Act 2005.
* Strong assessment and analytical skills.
