CARE COORDINATOR (HOME CARE)

Starting Salary:- £26K P.A. - £28K P.A

Bracknell, Berkshire

FREE PARKING, COMPANY PENSION

We are working with a highly regarded Home Care provider based in Bracknell, Berkshire, who is looking to recruit a passionate and motivated individual to join their growing team.

This is a fantastic opportunity to step into a rewarding Care Coordinator role, where you will play a key part in delivering high-quality care to clients within the local community. You will make a real difference to the lives of predominantly elderly individuals, supporting them to remain safe, independent, and comfortable in their own homes.

The company is currently experiencing an exciting period of growth, offering genuine opportunities for career progression into senior roles such as Deputy Care Manager and Registered Care Manager. However, they are equally keen to hear from candidates who are committed to developing and excelling within a Care Coordinator position and have a genuine passion for Home Care.

If you are either an experienced Home Care/Domiciliary Care Coordinator/Scheduler/Senior Carer, Field Care Supervisor or Care Team Leader this is a great opportunity to progress your career and be part of my clients successful journey!

Reporting to:- Registered Care Manager

Duties will include:-

* Care Coordination/Rostering/Scheduling
* Prioritize well being off both Care Staff and Service Users when scheduling
* Care Plans
* Risk Assessments
* Deliver a Safe & Compliant Home Care Service within CQC Guidelines, provide the utmost Customer Service to Services Users and their Families at all times
* Care Worker Spot Checks
* Administration
* Safeguarding of both Care Workers & Clients (Service Users)
* Liaise with Key Social Care Parties:- Social Workers, GP Practices, NHS Discharge Teams, CQC
* Deliver Personal Care to Clients (Service Users) in the community when

needed in order to comply with CQC regulations

* Be part of a rota'd Team On Call Service

Working Hours:

Office Based:

Monday - Friday 09:00hrs - 17:00hrs

or

Monday - Friday 08:00hrs - 16:00hrs

or

Monday - Friday -07:00hrs - 15:00hrs

On call rota service 1 weekend in 6 weeks

Desired experience:

Care Coordination/Rostering/Scheduling experience

Strong relationship building skills and empathetic nature

Home Care Experience (Carer)

Proficiency in following CQC regulations, Safeguarding Measures
