Following exciting changes in our services and staff progression, we’re recruiting tomultiple permanent and fixed term social work vacanciesacross our:
Hospital Discharge Team – play a vital role in supporting timely and safe discharges from hospital, working closely with health professionals, the person and their families, and community services to ensure person-centred outcomes
Learning Disability Team – empower individuals with learning disabilities to live fulfilling, independent lives. You will work collaboratively to identify outcomes, develop support plans and promote inclusion and wellbeing
Neighbourhood Team – be part of a locality-based team delivering holistic, strengths-based support to adults in the community. You’ll build strong local links and help people stay connected and independent
