Care Navigator
Our Care Navigation team is growing and we need you
The post holder will work within a large team of medical receptionists, liaising with GPs, nurses, healthcare assistants and administration staff.
Main duties and responsibilities will include:
* Dealing with telephone calls and managing face to face enquiries, booking appointments, directing patients to the right person and/or organisation
* Taking accurate messages and identifying the priority/urgency of the message
* Generating electronic repeat prescriptions and sending them to the GP for sign off
* Compiling new patient registration packs
* Undertaking duties as required by the Operations and Deputy Operations Manager
* To communicate in a professional, friendly and open manner with patients, carers and the general public to maintain an excellent profile for the Practice
* To communicate effectively and sensitively with service users who may be angry, confrontational, upset, anxious, fearful or have communication difficulties such as poor language skills, hearing loss or learning disabilities
* To support the effective maintenance and management of practice systems
* Maintain regular consistent and professional attendance, punctuality, personal appearance and adherence to relevant health and safety / environmental procedures
* Under the Data Protection Act 1998, the post holder must maintain the confidentiality of information about patients and staff. The work is of a confidential nature and information gained must not be communicated to other persons except in the recognised course of duty.
Candidate One: Working hours will be (24 hours):
* Monday: 8:30 - 6:30
* Tuesday: 8:30-6:30
* Friday: 1:30 - 6:30
Candidate Two: Working hours will be (24 hours):
* Monday: 8:30 - 6:30
* Tuesday: 1:30-6:30
* Friday: 8:30 - 6:30
When applying for either position you need to ensure you can do the hours advertised, we are unable to manipulate these times to suit individual circumstances.
Starting salary will be £12.50 rising to £13.05 after passing a three month probation period.
Due to the nature of our business we are unable to accept applications from candidates not already living in the UK.
