Overview
Adult’s Social Worker – Team: Neighbourhood, Location: Bolton. Bolton are looking for a SW for the Neighbourhood team; any age adult SW 18+ with any need other than LD. The candidate will work with health professionals such as district nurses and therapists. The role is hybrid (50/50 office and home) but requires ability to attend the office or complete a visit at short notice.
An exciting opportunity has arisen to join Bolton’s Adult Central South Neighbourhood team. You will work closely with health colleagues to provide support to adults and their carers who may be eligible for assessment and care services under the Care Act 2014.

Responsibilities
* Complete Care Act assessments and devise care and support plans; complete reviews.
* Complete Mental Capacity Assessments and participate in Best Interests meetings.
* Work with people with complex social needs and manage family dynamics, crisis, and legal issues; provide information and advice; transfer to another team as required.
* Ensure safeguarding function within the service, including assessing and investigating risk and undertaking Safeguarding Adult Enquiries as required.
* Attend case conferences, reviews, and represent the Council at Court when appropriate.
* Utilise resources from Social Care, Health and other agencies to assess the person’s ability to maximise independence and determine ongoing support needs.
* Provide ongoing support and maintain proactive engagement with individuals and carers.
Location, work arrangement and team
The role is located in Bolton, within the Neighbourhood team. Local candidates who can commit to Bolton are preferred. The role is hybrid, but candidates must be able to come into the office or complete a visit at short notice. Initially, candidates will be fully office-based for the first 4 weeks.
Requirements/Qualifications
* DIPSW or equivalent qualification in Social Work
* Social Work England registration
* Enhanced and in-date DBS certificate
* Excellent communication skills and ability to make good decisions based on available information
* Ability to work alone and within/contribute to a team/multi-agency setups
* Knowledge of service needs
* Competent IT skills and ability to navigate electronic file/recording systems
* Ability to write clear, concise, and robust assessment/reports/files
* Flexible approach to work and working hours when needed
