Exciting opportunities have arisen for new Community Rehabilitation Assistants to join Poole Intermediate Care Service.
Role Overview
The chosen candidates will work as part of a Multi-disciplinary Team caring for patients with an illness in their own homes to prevent hospital admissions and facilitate complex discharges from hospital. You will work alongside qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians, and wider locality teams. Good communication skills and a caring attitude are essential.
Working Conditions
Shift working is required as the service operates 365 days a year from 08:00 to 20:00. The successful candidates will need to work some weekends. Full or part-time permanent and temporary hours are available. You must use your car to visit patients at home; car owner/driver is essential, subject to the provisions of the Equality Act 2010.
Qualifications and Skills
* Basic IT skills
* NVQ Level 2/3 (advantageous)

Job Responsibilities
* Contribute to the aims of the service by working to prevent avoidable hospital admissions and support early discharges.
* Increase independence and reduce dependence on long-term health or social care, avoiding long-term care admissions where possible.
* Work as part of an integrated, multi-disciplinary team delivering outcome-based packages of intermediate care.
* Assist in planning, organizing, and delivering packages of care as directed by a qualified practitioner.
Additional Information
This position requires an Enhanced DBS check, which the Trust will cover. Applicants who subscribe to the DBS update service can present a valid DBS certificate. Applications requiring current Skilled Worker sponsorship are ineligible.
