Job overview

This is an exciting opportunity to work within the Flow Management Team. The successful candidate will be someone who thrives in a dynamic environment, able to juggle multiple priorities, stay calm under pressure and keep patient safety at the centre of their work. You will ensure that the patient is supported through their inpatient journey and safely supported throughout the discharge planning process.

The successful candidate will have the ability to meet deadlines, and a have a proactive approach to problem-solving whilst working with the wider wider care team.

The Purposeful Admission Coordinator will work agile and provide ward and services ‘in reach’ across the acute wards at LMH and ACMH.

Main duties of the job

To support the provision of purposeful inpatient admissions across the acute inpatient pathway and ensure the timely discharge of patients from the inpatient wards at Lynfield Mount Hospital (LMH) and Airedale Centre for Mental Health (ACMH), including the Step Forward Centre.

To commence discharge planning at the point of admission in collaboration with families/carers, inpatient MDT, Community Mental Health Teams (CMHT), Intensive Home Treatment Team (IHTT) and social care where appropriate. Undertaking appropriate assessments and challenging the MDT around purposeful admission utilising the Formulation and Care Planning process and identifying and escalating barriers to discharge using Criteria Led Discharge Tools.

The post holder will highlight any barriers to discharge to the senior management team and provide Delayed Transfer Of Care (DTOC) reports as required.

The post holder will be responsible for recording planned discharges and any barriers to discharges within the agreed electronic systems and liaise with key staff within The Trust to ensure that the position is communicated and understood. They will be responsible for escalating, where required, challenges around purposeful admissions and discharges in the relevant meetings and will work collaboratively with the wards to identify increased length of stay (LOS) eg. 100+ days within acute wards and 6 weeks LOS in PICU wards.

1. Relevant professional Mental Health legislation/guidance and Community Care Legislation and Guidance.

Experience

2. Direct and continuous relevant experiences at Band 7, or equivalent, which must have included the management and supervision of staff.

Desirable criteria

3. Experience in team leadership.

Qualifications

4. Current Registration on part 3 or 13 of NMC Register OR Diploma OT/BSc OT, Dip SW/AHMP, RN (Mental Health), or equivalent

5. Advanced Nursing qualification.
6. Management qualification.

7. Efficient, effective, and acceptable use of ward resources and staff.

8. Budget management skills.
