Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Adult Integrated Multi-Disciplinary Hospital Team to work Full Time based in Hull.
The salary for this Social Worker job is up to £37,938 per annum.
Key Responsibilities:
1. To promote and safeguard the welfare of adults at risk, families and their carers, within the 'making safeguarding personal' principles.
2. Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose.
3. Actively engage in team meetings/practice forums and other relevant meetings ensuring focused and purposeful social work is undertaken with all cases.
4. Work with colleagues in the team to ensure the form of communications are clear and aligned with people's needs.
5. Demonstrate effective use of evidence-based social work practice and ensure effective communication with people who have specific communication needs.
6. Ensure that case recording is up to date and that required information is available for a variety of purposes e.g. multi-agency meetings, strategy meetings, panels, court, case conferences etc.
7. Engage in the professional decision-making process with colleagues in relation to complex/high risk cases.
8. Provide co-working support and advice to other staff members within the team.
9. Actively support and engage in practice development groups and the provision of information/professional opinion as required.
10. Take a leadership role in managing the daily activity of the team, i.e. allocation, duty work.
Requirements of this Social Worker job:
1. A social Work Qualification or equivalent.
2. Understanding of relevant legislation.
3. Registered or eligible for registration.
