Band 8a Hub Manager – Autism Enhanced Support Team

* The post holder will be an experienced clinician or practitioner working alongside the Head of Learning Disability and Autism and will sit within the Corporate Nursing Directorate at BSMHFT.
* To lead in the provision of quality evidence-based practice, support and care for users of both services. The post holder will require specialist knowledge of the health and social care needs of people with learning disabilities, autism and/ or mental health as well as local and national priorities across health services and the impact of these on people with autism with or without mental health needs.
* The post holder will be expected to offer overall professional clinical leadership and management to all staff within the teams that make up the hub, to manage and coordinate staff and clinical activity and have overall responsibility for the professional development and supervision of practise
* To provide specialist practical advice on the care and clinical management of users of the services and to support staff, other agencies and other statutory services to provide integrated services to enable improved health and well being outcomes for service users.
* The successful candidates will also have highly developed reporting and analysis skills and be confident with having outcome focussed and collaborative conversations with a variety of stakeholders to promote patient outcomes and system challenge.


Main duties of the job

To be responsible for ensuring the appropriate communication, support and intervention takes place with their team, service users, families and other stakeholders

To ensure the service delivers advice, guidance and expertise to existing teams within BSHMT and provide care and support for those deemed at risk of admission to a mental health hospital bed. The post holder will offer intensive support to their team with safe and timely discharge into the community where required.

To be responsible for the development and maintenance of effective clinical pathways between an Autism Enhanced Support service within BSMHFT and other services (both internal and external). They will provide leadership and guidance to others in risk assessment, risk management and positive problem solving when supporting people with autism and co-existing conditions requiring intensive and crisis support and lead on debriefs following incidents.

To lead in the assessment, planning, implementation, and evaluation of care across the clinical services, and ensure that staff have a working knowledge of local, national policies and legislation which govern current service provision

Provide advanced clinical skills, leading on practice issues and providing leadership and management to teams supporting to develop highly specialised care packages for patients transitioning from in-patient areas and living within community settings.

Person Specification

* Registered health or social care professional with current up to date registration
* Post-graduate/qualifying registration experience with Band 7 experience
* Experience of working with individuals with autism and their families
* Proven track record of crisis, serious incident management in health care setting
* Demonstrable post registration experience of managing teams
* Proven record of evaluating nursing care/care delivery and driving up standards of practice.
* Experience of clinical governance / service improvement
* Ability to read and interpret data/information
* To be able to demonstrate motivational leadership skills and drive new initiatives
* Further professional registration or additional training relating to the needs of people with autism
* Experience of leading care and teams who provide care for people with autism or learning disabilities.
* Working knowledge of the local systems within health and social care organisations.
