About the role:
1. Work closely with the local authority and Streets to Home Partnership to enable rough sleepers to reconnect with their district of local connection
2. Establish contact with rough sleepers and beggars: working within Cambridge City protocols and partnerships to rapidly assist individuals through street outreach shifts at a variety of times and a range of locations in Cambridge City.
3. Establish good working relationships with other local authorities and develop an understanding of housing solutions that don’t require a local connection.
Identify and target Individuals who are rough sleeping, including those unwilling to engage with services by providing assertive outreach.
4. Work closely with drug and alcohol agencies to ensure that substance misusers and problem drinkers can smoothly transition their treatment to areas where they can receive associated housing support.
5. Work with Mental Health Agencies to ensure that where appropriate, service users have access to services to address their mental health issues and smoothly transition this care to areas where they can receive associated housing support.
6. Work proactively to ensure that service users have access to ongoing healthcare.
Communicate effectively with rough sleepers and complete referrals to accommodation outside of the city
7. To participate in all Bi-Monthly street counts.
What We’re Looking For:
8. Reconnections expert: Someone that brings an innovative approach to helping people reconnect to their local area where they will find the most appropriate level of support with their homelessness.
9. Experienced in Outreach and Community Support: A professional with a deep understanding of the challenges and nuances of homelessness outreach, including an awareness of social and systemic issues impacting vulnerable individuals.
10. Results-Driven and Compassionate: You know how to achieve meaningful outcomes, balancing a results-focused approach with a strong sense of empathy and understanding for complex personal circumstances.
11. Committed to Making a Difference: A true advocate for social change, passionate about helping others and driven by a mission to create long-term, positive impact.
