Job Overview
NHS Scotland is seeking a dynamic Occupational Therapy Assistant Practitioner to join the East Ross Discharge to Assess team. The successful candidate will work with individuals in their own home, leading on non-complex assessments on discharge from hospital and delivering targeted and intensive rehabilitation programmes to maximise patient independence.
This is an exciting opportunity to work within an established Integrated team in a community-based setting, focusing on facilitated discharge and prevention of admission. The ideal candidate will have the ability to manage a caseload, provide assessment, rehabilitation, onward referral, and discharge to a range of patients referred to the service.
Responsibilities
* Work with individuals in their own home, leading on non-complex assessments on discharge from hospital
* Deliver targeted and intensive rehabilitation programmes to maximise patient independence
* Manage a caseload, providing assessment, rehabilitation, onward referral, and discharge to patients
* Work within multi-professional teams to ensure co-ordinated care for patients
Requirements
* A full current driving licence and use of a vehicle are essential
* Ability to travel efficiently and effectively between various work locations and patient residences
