Job Summary
We are looking for an experienced ACP/Specialist Paramedic/Nurse Practitioner for our Home Visiting Team and practice-based clinics. Working alongside practice teams and another PCN-based ACP/paramedic, we are seeking team members who can bring the skills and experience to continue providing a high-quality service. The role includes home visits, so you will need to have access to a car and a clean driving license. We are seeking someone with a strong level of clinical care experience, who can multitask and remain calm under pressure. The post holder will provide assessment, differential diagnosis and treatments of acute minor health and minor injury conditions, including a home visiting service for appropriate patients, acting within professional boundaries.

Main duties
* See patients autonomously, providing face-to-face appointments, home visits and telephone consultations for patients within the Practice and in patients' residences; demonstrate critical thinking in clinical decision-making.
* Operate within your sphere of competence to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
* Prioritize and triage patient needs, making referrals for investigations as appropriate.
* Manage patient caseloads for treatments, referrals, admissions and discharges as applicable.
* Work collaboratively with the general practice team to meet patient needs, promote self-care and empower patients to manage their healthcare; supervision from a doctor is available if needed.
Clinical Practice
* Assess, diagnose, plan and implement care for patients with undifferentiated presentations.
* Clinically examine and assess patient needs from physiological and psychological perspectives and plan care accordingly.
* Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatments.
* Provide evidence-based practical skills in areas such as coughs and colds, sore throats, earache, skin rashes, diarrhoea/vomiting, soft tissue injuries, wounds, abdominal pain, urinary/vaginal symptoms, sexual health assessments and contraception.
* Prescribe and review medication within knowledge and guidelines; coordinate care to prevent unnecessary hospital admissions; support independence and adherence to treatment.
* Promote health and self-care; support sexual and family planning health needs where applicable.
* Liaise with patients, carers and colleagues to ensure home safety when needed; participate in audits and clinical governance.
Qualifications
* Essential
o Bachelor’s degree in a relevant program approved by the College of Paramedics or Nursing & Midwifery Council.
o Registration with the relevant professional body (e.g., HCPC or NMC).
o Full UK driving licence.
* Desirable
o Non-medical prescribing qualification (or working towards).
o Masters-level capability as Clinical Practitioner (or working towards).
Experience
* Essential: Experience managing a designated caseload with assessments, planning, interventions and outcome evaluation aligned to care plans; ability to work independently and in partnership with patients and carers; ability to write comprehensive clinical notes and use clinical coding; ability to determine pathways of care based on urgency and severity; risk management in unpredictable situations; adherence to policies and codes of conduct.
* Desirable: Experience in primary care.
