Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision-making, to establish a diagnosis and appropriate management plan. Practice within scope of professional registration (e.g. NMC). Undertake triage, (including online), for patients seeking access to services. Provide safe, evidence-based, cost-effective, individualised patient care, referring patients directly to other services/agencies as appropriate. Manage caseload and clinical priorities according to agreed protocols and working practices. Within scope of practice and clinical competence request and/or undertake diagnostic procedures and clinical investigations related to plans of care. Have a working knowledge of agreed national and local guidelines for clinical practice. Working as part of the wider practice team, utilise advanced knowledge and skills to provide advice and support to other members of the multidisciplinary team. Where clinically appropriate, implement and action changes within the patient medical record Liaise with doctors and other healthcare professionals in secondary care in order to provide the optimum care for your patient. Act as a Non-Medical prescriber, working within clinical competencies at all time, and to your Professional body, adhere to the Standards for Competency Framework and local and national guidance. Regularly attend clinical governance sessions and take part in audits of yours and others practice in order to maintain safe and effective practice. Use professional judgment to act as an advocate for patients and ensure a patient centred approach to the delivery of care. Support and enable patients and carers to make informed decisions relating to their treatment and management. Escalate any concerns via the nursing/professional structures are required. Ensure high standards of documentation. Records should be appropriate, complete and up-to-date and should adhere to the principles of the practice confidentiality and data protection policies. Occasional work out of hours to support vaccination campaigns, health promotion campaigns and other events within the University Work with the general practice teams to ensure the practice is compliant with relevant CQC standards. Work with your line manager to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present.
