Job Summary
We are looking for a caring, dedicated and reliable individual to join our team as a non‑clinical care coordinator supporting the wider pharmacy team. This is an important role within the PCN, focusing on using systems and data to proactively identify patients in need of routine drug monitoring, alongside coordination and navigation of care.
Main Duties of the Job
We rely on systems and data to proactively identify patients in need of routine drug monitoring, and coordinate and navigate care for them.
