Responsibilities
* Manage planned programmes of care for patients in domiciliary and clinic settings.
* Assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.
Our Community Nursing Team supports the needs of housebound patients living on the Wirral. We provide valuable care, supporting people to manage their long term conditions, and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they are admitted.
