Main duties of the job
As a General Practice Nurse at Medicus Health Partners, you will:
* Develop and implement health promotion and disease prevention programs.
* Manage and deliver treatment plans for patients with long-term conditions.
* Provide emergency care and support patients in urgent situations.
* Administer vaccinations and immunizations for both adults and children.
* Offer advice and support for family planning, mental health, and other health concerns.
* Conduct cervical cytology consultations and manage uncomplicated wounds.
* Collaborate with the general practice team to ensure comprehensive patient care.
* Utilise technology to manage patient data and support clinical decision-making.

Job responsibilities
Clinical practice
* Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
* Implement and evaluate individual treatment plans for patients with a known long-term condition.
* Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Qualifications
* RGN, current NMC registration.
* Qualification in at least one of the following chronic disease areas: asthma, COPD, or diabetes.
* Chronic disease management qualifications in additional areas (asthma, COPD, CHD, diabetes); childhood immunisation and cytology qualifications or willingness to train to achieve these qualifications; clinical skills qualification, degree level or equivalent; independent nurse prescriber.
