What You Will Do
* Lead and oversee the Hospital Discharge and Post‑Discharge Assessment Team, ensuring interventions are timely and high quality.
* Support seamless transition of patients from hospital to community settings.
* Provide guidance and supervision to team members, enhancing professional capabilities.
* Liaise with healthcare professionals and community services for coordinated discharge.
* Conduct regular reviews and assessments to maintain service excellence.
* Maintain accurate, comprehensive records in compliance with Social Work England registration requirements.
This position is ideal for an experienced Team Leader, Advanced Social Worker, or Senior Practitioner with a strong background in Adult Social Care and a firm grasp of hospital discharge and assessment pathways.
