* Conduct comprehensive needs assessments for adults requiring social work support.
* Develop, implement, and monitor care plans tailored to individual needs and circumstances.
* Collaborate with multidisciplinary teams, including healthcare professionals and family members.
* Provide direct support and advocacy to clients, ensuring their rights and well-being are protected.
* Maintain accurate and up-to-date case records, adhering to professional standards and regulatory requirements.
* Participate in case reviews and contribute to the ongoing development of service delivery.
* Network and liaise with relevant agencies and organizations to facilitate holistic care.
* Contribute to the overall improvement of social work practices and service quality.
Expectations / Requirements
We're looking for you, if:
* You hold a relevant social work qualification (e.g., MSW, BA in Social Work).
* You possess a minimum of 2 years of post-qualification experience in adult social work.
* You have experience working with adults with complex needs.
* You demonstrate a strong understanding of relevant legislation and policies.
* You possess excellent communication, interpersonal, and problem-solving skills.
* You are a highly motivated and compassionate individual with a commitment to client-centered care.
* Experience as an AMHP is highly desirable.
